Please note that the following charges will apply from the 1st March 2023 for patients requesting any documentation from a doctor.
You can pay by cash or debit card in the surgery.

Charges apply for any reports requested from a GP as follows:

| Child Minder Application (Ofsted) | £90.00 |
| HGV / LGV Materials (Incl. VAT) | £150.00 |
| Disabled Bus Pass | £25.00 |
| Police Medical Questionnaire | £120.00 |
Important Notes –
- Payment of these fees are required on request of the documents (which will not be released until payment is made).
- A receipt will be issued on request.
- You will be notified when the documents are ready to collect.
