Please note that the following charges will apply from the 1st March 2023 for patients requesting any documentation from a doctor.
You can pay by cash or debit card in the surgery.

Important Notes –
- Payment of these fees are required on request of the documents (which will not be released until payment is made).
- A receipt will be issued on request.
- You will be notified when the documents are ready to collect.
