A Travel Risk Assessment Form is available from Reception or online. Please complete and return to the surgery (at least eight weeks prior to departure) to enable us to ascertain which vaccinations (if any) are required.
Prior to the nurse contacting you regarding your trip, we advise you to look on TravelHealthPro website, regarding the vaccines and risks for the country you are visiting.
Vaccines that are provided under the NHS by our nursing staff, within the practice are Tetanus, Diphtheria, Polio, Hepatitis A and Typhoid, also Cholera.
These vaccines are free of charge.
There may be other vaccines that you require for your trip. Some of these are a course that will need to be given over a number of weeks. These will have to be given at a private travel clinic and are chargeable.
