Please be aware we do not take prescription requests over the phone. you can request your prescription the following ways:
- Signing up for our online services with one of our care co-ordinators. This requires you to complete an online services form and provide photo ID – Useful for accessing your online records and any recent results.
- Emailing our prescription admin team/ Dispensing team directly on [email protected]. We aim to respond to all emails within 48 working hours.
- Dropping off your prescription request to our care co-ordinators at Garforth medical centre or Elmwood surgery.
