How To Register As A Patient

Patients will only be accepted if they reside in the practice area.

If you have a medical card please bring it to the surgery; if not, please call into the surgery and pick up the registration forms for completion. We will need to see proof of address and identification. Once registered you will need to make an appointment with the nurse/health care assistant for a new patient registration health check.

How To See Your Doctor

Patient records are now computerised therefore you can see a doctor at any of our three sites if necessary. You may like to see the same doctor at each visit so that you get to know each other; we shall try to arrange this, but unfortunately this may not be possible at short notice.Sometimes you will be asked if you would like to see a doctor other than one of the partners. This doctor will either be a locum who is helping us when we are busy or a general practitioner registrar.

Garforth Medical Centre

There is an appointment system at Garforth. The surgery is open from 8.00am to 8.00pm on Mondays, 7.15am to 8pm on Tuesdays (with the exception of the second Tuesday in the month, when we will be open these hours the following day), Wednesday 8am to 6pm (except Wednesday that follows the second Tuesday), Thursday and Friday 8.00am to 6.00pm. A nurse is always in attendance when the surgery is open. The doctors' surgeries are staggered throughout the days with the earliest appointment at 7.20am and the latest at 7.30pm.

Elmwood Surgery

There is an appointment system at Barwick. The surgery is open from 8.30am to 5.30pm on Mondays, 8.30 am to 12 noon on Tuesdays and 8.30am to 5pm Wednesdays, Thursdays and Fridays. Additionally the surgery is open 8.30 am to 11.30 am on the first and third Saturday in the month (for pre-booked appointments only). The surgery is closed between 12 noon and 2.00pm everyday. The first appointment with a doctor is at 8.40am and the last at 4.40pm (5.20pm on Mondays). A nurse is always in attendance when the surgery is open.

Jessamine Cottage Surgery

There is an appointment system at Aberford. The surgery is open from 8.15am to 12.15pm Monday to Friday with a nurse always in attendance when the surgery is open. The first appointment with a doctor is at 8.30am.

Home Visits

These are for people who are physically unable to come to the surgery. If you require a home visit please try to call us between 8.00 and 11.00am. It is very helpful if you can give us some idea of the medical problem involved as then the patients needing the most urgent calls can be visited quickly.

During the week you will usually be visited by one of the partners or the GP registrar. At weekends and at night you will be visited by a doctor from the deputising service.
